File a Complaint

Before the Commission considers your claim, you must contact your broker and receive a formal response to your claim. According to the commission rules, the broker must provide an answer to your claim within 14 days.

Provide a reference number so that your financial service provider can find the information you need.

Describe the nature of your complaint. Please include as much information as possible, including the type of product, dates of the problem, name, address and telephone number (if known to you) of all persons and companies involved and a description of the problem (please enclose a separate page if you need more space than indicated).
